    We’ve been talking about loss of teeth and the horrible thought of being fitted with a plate.     I am Belinda Rudd and I am with some of the doctors from the All-On-4 Clinic, where for many years now, they have been transforming smiles and helping change the lives of many people with their advanced dental implants techniques.

    Dr. Fadi Yassmin: Dental implants are titanium fixtures that are inserted in the jaw bone under the gums to serve as anchors.

Cheap All-On-4 Teeth Replacement Comes at a Price

    Dr. Hillel New: You can see here an all-on-4 style bridge done elsewhere, using cheaper methods. You can see the large plastic flanges of the denture and repairs that were done in the process of attempting to retro-fit it to the implants. Because the under-surface is rugged, it is impossible to keep clean. If you can not clean it, this is what accumulates under the alternative styles of All-On-4.

    And this leads to inflammation around the implants and long term problems. Whereas this shows you the All-On-4 Clinic method of producing the final teeth. In our processing of the bridge there are no plastic flanges. The interface is flat, which allows the patient to keep clean both the teeth and gum areas for improved health of the gums and implants.

    In this picture you can see from the inside that the upper teeth upper teeth were retrofitted. In other words, the denture that was made was cut back and holes were cut in it in the positions of the implants. After metallic cylinders were attached to the implants, the dentist connected the denture to those cylinders with a direct self-curing resin.

    You can see this lighter colour around the metallic cylinders. Another issue here is that there is still partial coverage of the palate by the denture. Also, the positioning of the front two implants is not ideal causing bulkiness from the inside, and this can often be uncomfortable or cause speech issues, even when the next set of teeth are made some months later. A similar problem is seen in the lower of this same patient, and you can also see the rugged under-surface which makes it very difficult to clean.

    In contrast, this is the All-On-4 Clinic method shown here in the lower jaw. Not only do we produce the final bridge within 24 hours, but again our technique is also designed with flatter contours to simplify cleaning. I keep repeating the cleaning because its important for us to make it simple for our patients.

    When you compare x-rays, on the left is an x-ray of an All-On-4 using alternative methods and on the right you can see that our technique has a rigid bar connection between the implants. This make the bridge more durable. These are just some examples of how we do it differently at the All-On-4 clinic.

    Presenter: Personally, I am not one to gamble when it comes to my health. I didn’t realize there were such differences between the methods.     Presenter: Unusual as this may sound, I was surprised to learn that there is no registrable specialty actually dedicated to dental implants.

    Dr. Misagh Habibi: Its unusual that there is no actual specialty restricted to dental implantology, so it’s important to know who you are actually seeing.

    The advice I would give anyone who is investigating dental implant teeth replacement is to see a dentist who does these procedures routinely on a daily basis. Ask them about their surgical & anaesthetic experience, ask them about their qualifications as a leader in dental implantology, ask them about how many cases they’ve successfully performed.

    Presenter: Recently, wherever I turn I see dental advertisements for All-On-4 Dental Implants treatment. I’m Belinda Rudd and I’m here with some of the doctors from the All-On-4 Clinic. So what is All-On-4, and is there a difference between various methods and the cost?

    Dr Alex Fibishenko: “All-On-4” is a treatment concept that has evolved over the past 20 years involving the placement of four Titanium fixtures into either an upper or lower jaw and then placing a whole set of teeth on them. In many cases, the treatment can be completed within a day or so.

    Dr Misagh Habibi: In the usual approach to the all-on-4 method, patients receive a temporary acrylic set of teeth after their surgery and are told to return in a few months to have a more aesthetic and a stronger set of teeth made, which often has a titanium foundation. At All-On-4 Clinic we have developed a unique methodology where the immediate set of teeth that patients receive is actually the permanent set, more aesthetic more comfortable and stronger.

    Dr Alex Fibishenko: One of the other problems with techniques involving the fitting of a temporary bridge, apart from being unreliable and difficult to clean, is the potential long-term affect on the health of the dental implants and bone loss. The main concern is that this treatment, when the problems become known, is complex to undo and often impossible to re-do.

    Presenter: So what would be the cost of All-On-4 Dental implants treatment?

    Dr Helen Arabatzis: Typically, with All-On-4 Clinic’s methods it is on average $23 to $27 thousand dollars. This includes the work-up procedures, the surgery, the fitting of immediate and final teeth, the reviews and x-rays at 3 weeks and 4 months.

    Dr Hillel New: We use genuine Swedish ‘Nobel Biocare’ Implants and components that makes a big difference to the way the treatment is performed, as it was the Swedish company, Nobel Biocare that introduced the required components for the All-On-4 concept back in 2004.
